Documentation of the NCP
There are a variety of chart-entry formats in use by dietitians, such as:
- Assessment, Diagnosis, Intervention, Monitoring and Evaluation (ADIME)(1)
- Anthropometry, Biochemistry, Clinical, Diet (ABCD)
- Diagnosis, Assessment, Plan (DAP)
- Data, Action, Response (DAR)
- History, Observation, Assessment, Plan (HOAP)
- Problem, Intervention, Evaluation (PIE)
- Subjective, Objective, Assessment, Plan (SOAP)
The first of these—ADIME—most closely mirrors the NCP, and this format is compatible with any charting or documentation system, electronic or manual. However, any charting method can be utilized with the NCP process.
